Shah is the President of South Texas Wound Associates, where he provides clinical wound care services in San Antonio since past 22 years; President of TIMEO2 Healing Concepts, LLC, which provides consulting and education services in wound care and hyperbaric medicine; Medical Director of Wound Healing Center at Northeast Baptist Hospital; Co-Medical Director of Mission Trail Baptist Wound Healing Center in San Antonio, Texas; current President of American College of Hyperbaric Medicine; Past President American Association of Physicians of Indian Origin; Past President of Bexar County Medical Society; Current Board of Director for ISTAP,current Board of Trustees Member of Texas Medical Association and AMA Delegation from Texas He has published “Wound Care Certification Study Guide” in 2011, its second edition in 2016; Textbook of Chronic Wound Care: An Evidence-Based Approach to Diagnosis and Treatment in 2018, and created “WoundDoctor” App, a comprehensive woundcare resource tool for physicians, nurses and physical therapists for iPhone, Android and iPad. The purpose of this lecture is to review the pathophysiology of idiopathic sudden sensorineural hearing loss (ISSNHL), avascular necrosis (AVN), and central retinal arterial occlusion (CRAO). The lecture will provide an up to date review on the use of HBO2 for the treatment of ISSNHL, AVN, and CRAO. Additionally, we will look at UHMS recommendations for treatment, treatment tables, time to treatment, and utilization review for these diseases. All relevant research will be reviewed. Dr. Weaver received a BS in Engineering Science from Arizona State University followed by medical school from the University of Arizona. He served a rotating internship in the US Navy, then went through the medical officer’s course in Undersea and Submarine medicine and was an Undersea Medical Officer on the USS Canopis (AS-34) for two years. After discharged he served in the Naval Reserves with SEAL Team 1-3-5 for a few years. After discharge from Active Duty he trained in Internal Medicine, with fellowship training in pulmonary and critical care at the University of Utah. After completion of fellowship he became the Medical Director of Hyperbaric Medicine and Co-director of the Shock Trauma Respiratory ICU at the Level One trauma center, LDS Hospital, Salt Lake City, Utah for 20 years. He was a co-investigator for the ARDSnet of the NIH and was the principle investigator of a randomized controlled trial of hyperbaric oxygen for acute carbon monoxide poisoning, published by the New England Journal of Medicine. He is a former president of the Undersea and Hyperbaric Medical Society. He has authored and co-authored hundreds of papers, abstracts and book chapters. For 10 years he had a senior leadership role in the DoD effort to conduct blinded randomized trials of hyperbaric oxygen for post-concussive syndrome due to War-related mild traumatic brain injury. Research activities include study of neural auto-antibodies following carbon monoxide poisoning and a randomized trial of hyperbaric oxygen for sequelae months to years after brain injury. He enjoys off-road UTV and adventure motorcycle riding, SCUBA diving, and spending time with his family.

Marc Robins, DO, MPH, FUHM is the Senior Medical Director for the Wound & Hyperbaric Shared Services for the Intermountain Healthcare System in Utah. He is the current President of the UHMS and has served as Co-Chair for the Quality Utilization, Authorization and Reimbursement Committee and on the Education Committee for the UHMS. He is an Associate Fellow of the Aerospace Medical Association (AsMA) and has served on the Science and Research committee AsMA since 2001-20. He is boarded in Hyperbaric Medicine, Aerospace and Occupational Medicine and Family Practice and retired as a Colonel from the USAF after a 20-year career in Aerospace Medicine.

After growing up in Michigan, Dr. Kelly went to medical school at Ross University, on the island of Dominican the West Indies. He did his residency in emergency medicine at Charity Hospital, Louisiana State University in New Orleans. Dr. Kelly did a fellowship in Undersea and Hyperbaric medicine/Wound Care at Charity Hospital, LSU finishing in 2004. After his fellowship training, he spent several years in the San Francisco/Bay area working at several hospitals working in emergency medicine, and hyperbaric medicine/wound care. He missed academic medicine, and was recruited to come to the University of Alabama at Birmingham to initiate a hyperbaric and wound care program in 2008. The hyperbaric and wound care program at the University of Alabama was initiated 2012. In 2014 he was recruited to the University of Pennsylvania, and he spent 5 years acting as Chief of Undersea and Hyperbaric medicine at the University of Pennsylvania. In 2020 he decided to return back to UAB. Since that time, he has been acting as Co-medical director of the Hyperbaric and Wound Care program. He also work in the Emergency Department at UAB and has been active in developing the Snakebite Program at UAB.

Susan Churchill, APRN-NP recently retired after 26 years experience as an APRN in Hyperbaric Medicine at Intermountain Healthcare working with Dr. Lindell Weaver in the Hyperbaric Medicine Deparatments at LDS Hospital and Intermountain Medical Center in SLC, Utah. She had extensive clinical experience prior to her tenure in Hyperbarics (oncology, critical care and liver transplant), followed by certification as a nurse practitioner in 1996. Shhe was the first non midwife adult nurse practitioner seeking a role in the hospital setting at Intermountain Healthcare. While working with the hyperbaric department, she gain extensive experience with monoplace chambers, including critical care in the monoplace; care of patients in the multiplace chamber; and conducting research (including being the principle investigator of brain injury trial and as a research coordinator/co-investigator in the DOD sponsored BIMA study).

Taxes Surcharges/Fees
National Forest Service Donation– Call in Groups
A tax-deductible donation of $1 per room, per night will be added to your individual group guest’s lodging bill to support the National Forest Foundation (NFF), a non-profit partner of the U.S. Forest Service, to support local forest conservation. For every dollar contributed by CMR lodging guests, the NFF will invest $1.50 in on-the-ground conservation projects in the Dillon Ranger District of the White River National Forest. This donation is voluntary. The payee may request Group Reservations Specialist remove the fee at time of booking.
Individual Lodging Deposit Policy
25 % of the total lodging amount will be required as deposit upon making reservations.
The balance on the room reservation will be billed to the credit card number on file 30 days prior to check-in. Full payment is required at time of booking for reservations made inside of 30 days of arrival.

For individual attendees who cancel their lodging reservations, the following will be in effect:
Pre/Post Group Rates
Pre & Post stays are available at the group rate Sunday - Thursday. Weekend rates may be subject to increase due to winter season demand. All pre/post reservations will be accepted on a space available basis at the time of reserving room.
No-shows
Rooms which have been reserved and whose attendees fail to show on their scheduled day of arrival will be billed to the entity guaranteeing the reservation for the total lodging amount for the contracted time.
Early Departures
A fee of 100% of the remaining room nights lodging amount will be charged if an attendee checks out before their confirmed departure date. This may be waived for emergency situations but must be authorized by PCM.
Check In and Check Out Times
Check in time is 4 PM Mountain time. Check out is 10 AM Mountain time.
Requests for an early check in or late departure for a hospitality/change room cannot be guaranteed.
Parking
Complimentary parking is available to PCM lodging guests.
Group attendees not lodging in PCM managed properties may be required to pay daily parking fees for close-in parking. Free parking is available in the Alpine Lot by the entrance of the resort, which offers shuttle service to all villages.
ADA Compliance
The unit you will be staying in is an individually owned condominium and is not part of a hotel. Because all condominium units are individually owned and are not part of a hotel, units meeting the accessibility requirements for persons with disabilities may be unavailable or limited in number. However, all of PCM’s conference facilities and banquet venues are equipped to enable access to guests with disabilities.
Athletic Club
Guests lodging with PCM will have complimentary access to the athletic club amenities. These amenities include: cardio room, indoor hot tub, indoor pool, locker rooms and a weight room. Facials and massage are available at an extra cost.

IN RESORT TRANSPORTATION
Complimentary transportation will be provided for travel within the Village on a set schedule that varies according to season. Transportation requests outside of scheduled hours or for private group use will be assessed an additional fee.
